4 County
Community Care (4CCC) program

In partnership with
Duke University Medical Center, FCHN administers the 4CCC program. The
four counties included in 4CCC are Vance, Granville, Warren and Person
counties. 4CCC is a part of a statewide program called Community Care
of North Carolina (CCNC). 4CCC has been in operation since 2004. 4CCC
and Durham Community Health Network (DCHN) are “sister networks” that
together comprise Northern Piedmont Community Care (NPCC).
As a CCNC network,
4CCC is expected to manage the cost, quality, and access of healthcare
services delivered to the target population (approximately 18,000
Carolina Access/Medicaid recipients in the four county area) through
various means. The 4CCC health programs include asthma and diabetes
disease management programs, coordination of needed healthcare services,
health education services, and encouragement of utilization of the
primary care physician as the “medical home” for Carolina Access
recipients.
4CCC has a local
advisory committee that includes representatives from the area primary
care physician practices, local hospitals, local Health Departments, and
local Departments of Social Services.
